Forehead lines show up for two very different reasons, and knowing which one applies to you is the first step in choosing the right treatment.
Why Forehead Lines Form
Dynamic lines appear only when you move your face — raising your eyebrows, expressing surprise, or simply talking. These come from repeated contraction of the frontalis muscle over years. Static lines, on the other hand, are visible even when your face is completely relaxed. These form later, as collagen and elastin naturally decline and the skin loses its ability to spring back after each muscle movement.
Most people have some combination of both, which is why a single approach rarely solves the whole picture.
Treatment Options, From Least to Most Involved
Neuromodulators: Botox and Dysport
For dynamic lines, Botox and Dysport are the first line of treatment. Both temporarily relax the frontalis muscle so it can no longer create the repetitive folding that causes the lines. Results typically appear within a few days to a week and last three to four months. This is the most effective option when your lines mostly disappear when your face is at rest — it’s treating the muscle movement causing them, not the skin itself.
Dermal Filler for Deeper Static Lines
When lines are visible even at rest, a neuromodulator alone often isn’t enough — there’s simply a groove in the skin that needs to be softened. A conservative amount of dermal filler can fill in that groove directly, working alongside Botox or Dysport rather than replacing it.
Skin Quality Treatments for Texture
If the forehead area also shows broader texture changes — thinning skin, fine crepiness, or a loss of overall firmness — treatments like CO2 laser resurfacing or RF microneedling address the skin’s structural quality rather than any single line. These stimulate collagen production across the whole treated area and are typically considered once the dynamic component is already under control.
How to Choose
The honest answer is that it depends on what your lines are actually doing. If they vanish when your face relaxes, start with a neuromodulator. If they’re still visible at rest, you’ll likely want filler alongside it. If the surrounding skin has lost overall quality — not just isolated lines — a resurfacing or microneedling treatment rounds out the plan.
Because every forehead is different, this is exactly the kind of thing worth discussing in person. Our team can look at your specific pattern of movement and skin quality and build a plan that addresses the actual cause, not just the visible line.
